Clear Aligners: How They Work, What They Cost, and Why Supervision Matters
Clear aligners are now the usual way adults straighten their teeth. They’re a series of transparent, removable trays, each worn for one to two weeks, each shaped fractionally differently from the last, so that over the course of treatment your teeth are guided into a planned position. No brackets, no wires, and nothing that stops you eating what you like.
Before you spend anything, it’s worth knowing that not all aligner treatment is the same. The trays may look identical. What differs is whether a dentist examined your mouth first, and whether anyone is watching what happens next.
How the treatment works
Treatment starts with a 3D scan of your teeth. From that scan your dentist plans the movement tooth by tooth and produces a digital simulation of the predicted end result, which you see on screen before agreeing to anything. Once you approve the plan, the aligners are manufactured as a full set.
You wear them 20 to 22 hours a day, removing them to eat, drink anything other than water, and brush. Small tooth-coloured attachments are usually bonded to a few teeth to give the aligners something to grip. These come off at the end. You change to the next aligner in the series roughly every week or two, and see your dentist periodically so progress can be checked against the plan and corrected if a tooth isn’t tracking.
Treatment length varies with the case. When the teeth are in position, you wear retainers to hold them there, at night, for good. Teeth drift back without them, and any provider who doesn’t make that clear isn’t telling you the whole story.
Dentist-supervised versus mail-order
Direct-to-consumer aligners, ordered online from an impression kit posted to your home, cost less for a reason: nobody examines you. That matters more than it sounds.
Before any tooth is moved, someone needs to establish that your gums are healthy, that there’s no untreated decay, that the roots and bone can take the movement, and that your bite will still work at the end. Moving teeth through unhealthy gum tissue can cause lasting damage. A general dental examination and, where needed, X-rays are what rule that out, and a posted impression kit can’t do any of it.
The General Dental Council has been clear that a dentist must carry out a clinical assessment before orthodontic treatment begins, and remain responsible throughout. When treatment is supervised, a tooth that stops tracking is spotted at a review appointment and corrected. When it isn’t, the first anyone knows is at the end.

The worry isn’t the plastic, it’s the missing examination. Without a clinical assessment, untreated decay or gum disease can go unnoticed while teeth are being moved, which risks lasting harm. A dentist should examine you before treatment and monitor it throughout.
How long do I have to wear retainers?
Indefinitely, at night. Teeth have a natural tendency to drift back towards their original position, and retainers are what prevent it. This applies after any orthodontic treatment, fixed braces included.
Can I drink coffee with clear aligners in?
Take them out for anything other than water. Hot drinks can distort the plastic and coloured drinks will stain it, and any liquid trapped against the teeth under an aligner raises the decay risk.
