Fixed braces are not obsolete, and anyone who tells you otherwise is selling something. For certain complex movements — significant rotations, large bite corrections, some extraction cases — fixed appliances still do things aligners struggle with, and a good clinician will say so.

What aligners give you is removability and discretion. You take them out to eat, so nothing is off the menu; you brush and floss normally, so hygiene is easier and the risk of decal­cification marks around brackets disappears. For the great majority of adult cases — crowding, spacing, drifting, mild-to-moderate bite issues — they do the job well.

At-Home Aligners

This is the one to think hardest about. The at-home companies are cheaper, and there’s a reason: you never see a dentist. Nobody examines you before treatment starts.

That matters more than it sounds. Moving teeth puts force through the bone and the gum. If you have undiagnosed gum disease — which is common, often painless, and something you can easily have without knowing — applying orthodontic force can accelerate bone loss and, in the worst cases, loosen teeth. If you have an untreated cavity, an infected root, or a bite problem that needs addressing first, an app will not spot it. A dentist will.

There’s also the question of what happens when something doesn’t track — when the tooth doesn’t follow the plan, which is normal and happens in plenty of cases. In practice, someone needs to look in your mouth and adjust. That’s the service you’re paying the difference for.

Invisalign isn’t magic. Most cases need attachments — small tooth-coloured bumps bonded to the teeth to give the aligners something to grip. They’re visible up close. Many cases need IPR: a fraction of a millimetre of enamel polished between teeth to create space. It’s painless and safe, but you should know it’s part of the plan. And the whole thing depends on you wearing them 20–22 hours a day. Aligners in a drawer move nothing.
