The bridge problem

A conventional bridge replaces the missing tooth by anchoring a false one to the teeth on either side. To do that, those two neighbouring teeth — which may be perfectly healthy — are drilled down to stumps and crowned.

That’s the trade. You get a fixed tooth relatively quickly, at the cost of permanently damaging two healthy teeth. And bridges don’t last forever; when one fails, you now have a three-tooth problem rather than a one-tooth problem. It’s a reasonable option when the neighbouring teeth are already heavily filled or crowned — then you’re not sacrificing anything. It’s a poor option when they’re pristine.

The denture problem

A partial denture is removable, cheap and quick, and it doesn’t touch the neighbouring teeth. It’s also plastic in your mouth, it needs taking out, the clasps can be visible, and most people find they don’t chew with it properly. Plenty of the dentures we see live in a drawer.

An implant replaces the root. A small titanium post is placed into the jawbone; the bone fuses to it over about three months; a crown goes on top. The neighbouring teeth are never touched.

The underrated part is the bone. When a tooth root is lost, the bone that held it begins to shrink away — it has nothing to do. That’s why long-term denture wearers develop a collapsed, aged appearance around the mouth. An implant loads the bone the way a real root did, and that recession largely stops. When an implant isn’t the answer

Sometimes it isn’t. Uncontrolled gum disease has to be treated first — placing an implant into an unhealthy mouth is asking for failure. Heavy smoking substantially raises the failure rate, and we’ll be blunt with you about that. Some cases need bone grafting first; others are better served by a bridge.
