The crown on top is a different matter. It’s a manufactured object under enormous repeated load, and it may need replacing at some point in a long life. That’s maintenance, not failure, and it’s worth budgeting for mentally.
What actually causes implants to fail
Four things, in rough order of importance:
- ✓ Peri-implantitis. The big one. It’s gum disease around an implant: bacteria, inflammation, and progressive loss of the bone holding the implant in. It is largely painless until it’s advanced, and it is the leading cause of late implant failure. It is also largely preventable with cleaning and regular hygiene appointments — which is precisely why we won’t place implants into a mouth with untreated gum disease.
- ✓ Smoking. Materially raises failure rates, both early and late. We will tell you this plainly rather than take your money and hope. If you’re going to stop, before implant surgery is the moment.
- ✓ Uncontrolled diabetes, which impairs healing and the bone’s ability to fuse to the implant.
- ✓ Grinding and clenching. An implant has no ligament, so unlike a natural tooth it can’t feel excessive force and doesn’t have that shock absorption. Heavy bruxism can overload the restoration. A night guard is often part of a properly planned case — and is often quietly omitted from cheaper plans.
